It is best to contact your local District Chairman. He or she will direct you to the current registrar for your area. You will be required to complete (at registration) a declaration of Residency form** indicating your new address and date of the move to the new address. The new association will also ask you for proof of address, i.e. copy of deed, copy of rental agreement, etc. If you are moving into the ODMHA from another Branch, i.e. moving to ODMHA from Toronto, Quebec, Winnipeg, etc. you must have a letter from your previous association stating that your child has played in their association the past season and is moving to the ODMHA without any prejudice, i.e. no outstanding fees or suspensions.
